The care pathway begins with standardized screening, typically performed during the second trimester, with earlier testing for high-risk individuals. Early identification allows prompt intervention before complications develop. Once diagnosed, patients are enrolled into a structured care pathway involving obstetricians, endocrinologists, dietitians, and diabetes educators.

Nutritional therapy forms the cornerstone of management. Individualized meal planning focuses on balanced carbohydrate distribution, appropriate caloric intake, and glycemic stability. Regular physical activity, when medically appropriate, enhances insulin sensitivity and supports glucose control. Blood glucose monitoring is implemented to track fasting and post-meal values, guiding treatment adjustments.

When lifestyle measures are insufficient to achieve target glucose levels, pharmacological therapy is initiated. Insulin remains the gold standard due to its safety and efficacy in pregnancy. Fetal growth and maternal health are closely monitored throughout pregnancy. The care pathway includes regular assessment of fetal size, amniotic fluid levels, and maternal blood pressure. Postpartum follow-up is a critical component of the care pathway. Although glucose levels often normalize after delivery, women with gestational diabetes have a significantly increased risk of developing Type 2 diabetes later in life. Postpartum glucose testing, lifestyle counseling, and long-term metabolic surveillance are therefore essential.
